AFMC’s Asthma Project works to improve care for asthma patients across the state. The project focuses on three main goals to decrease asthma emergency room use and hospitalizations:
- Increase follow-up visits for patients previously hospitalized with asthma complications
- Help patients and their families recognize the potential for a normal life with appropriate asthma therapy
- Increase physician recognition of chronic asthma and treatment of the condition with anti-inflammatory medications
Indicators
- Anti-inflammatory medications for mild persistent to severe asthma
- Regular use of objective peak flow monitoring
- Asthma Action Plan written and placed on each asthmatic chart with copy given to patient and caregiver
